ChartSpan partners with HAPevolve to bring value-based care to Medicare patients

GREENVILLE, S.C. - Amzeal -- ChartSpan, a value-based care organization, has partnered with HAPevolve, a subsidiary of The Hospital and Healthsystem Association of Pennsylvania (HAP), to offer preventative care programs to Medicare beneficiaries.

With Chronic Care Management services, Medicare patients with multiple chronic conditions can benefit from monthly check-ins, personalized health goals, and a 24-hour nurse line. ChartSpan's Annual Wellness Visit software also makes it easy for Medicare patients to complete Health Risk Assessments before meeting with their providers.
